The term "parathread" is being retired in favor of "on-demand parachain". This should be reflected in the implementers guide and code comments to prevent confusion.

Also will replace some outdated uses of the term "module" in favor of "pallet".

It would be nice to replace parathread in variable names as well, but that can be left as a separate issue. For now I'll at least leave comments addressing the renaming on each parathread variable definition.
