src: fix noexcept control flow issues

- adapt `NAPI_THROW`, create `NAPI_THROW_VOID`: either throw or
  return, never continue
- fix `Error::ThrowAsJavaScriptException`: if `napi_throw` fails, either
  explicitly throw or create fatal error
